TRAPP PROGRAM DETAILS

Referral: Referrals to this program will be made by any agency engaged with the child or the child’s family. Upon receiving the referral, the Program Coordinator will assign the client to a Facilitator. 

Case Management 

Within 72 hours of receiving the referral, the Facilitator will make initial contact with the client and the client’s family. The initial intake and behavioral evaluation will take place within 5 working days of receiving the referral. During the initial intake, the student will be accompanied by a parent/guardian who will then complete and sign the following forms (including, but not limited to): 

  • Client Information Sheet 
  • Consent to Treat 
  • Release of Information 
  • Schedule of Fees 
  • Privacy Policy 
  • Intake Assessment 
  • Biopsychosocial Assessment 

The Facilitator will schedule a Behavioral Health/Substance Abuse Screening, an Educational Screening, and a home study screening within the following 5 working days 

The Facilitator will act as a liaison between the referral agency, the parent/guardian, and The Bridge Center Evaluation staff. The Facilitator will be comprehensive, flexible, responsible, and persevering. He or she will seek to view and engage the children in the context of their families, deal with the families as part of the community, and have long-term preventative orientation, a clear mission, and continue to evolve the case plan over time. 

Behavioral Health/Substance Abuse Screening

A counselor (or intern) licensed by the Bureau of Alcohol and Drug Addiction will meet with the client and perform a Substance Abuse Subtle Screening Inventory (SASSI), and the Practical Adolescent Dual Diagnosis Interview (PADDI). The Counselor will score the above evaluations and, in communication with The Bridge Center’s Clinical Supervisor, design an individualized treatment plan, if necessary. 

Educational Screening

A State of Nevada Licensed Teacher will perform the following evaluation in order to assess each student’s educational IQ and developmental achievement: Weischler Individual Achievement Test 2nd Edition (WIAT-2). Testing areas will include but are not limited to: reading proficiency, math proficiency, listening comprehension, written expression, and oral expression. Based upon the child’s achievement and scores of the initial evaluations, further testing will be performed as necessary. Educational plans will be designed for each student in response to the results of the evaluations, in order to address areas of need and present challenges in areas of proficiency. Working in collaboration with the Washoe County School District, information will be shared to complete the student’s Individualized Educational Plan, if applicable. 

Home Study/Social Screening

The Facilitator will begin the social process by completing a home study. The tool used will be the Needs And Strengths Assessment (NASA). With supervision by a Licensed Social Worker, this home study will assist the facilitator in recognizing issues in the home that may need to be addressed. Any and all areas found to be of need will be addressed instantly with a referral(s) to appropriate agencies (i.e. Federal and State Welfare, Children’s Cabinet, Emergency Granting Agencies, and Food Bank, etc…) The MSW will continue to meet with the family, ensuring that all needs are eventually met. 

Individualized Case Plan

At the conclusion of the evaluation process, the Facilitator will organize a staffing including: the client, the parent/guardian, the Behavioral/Substance Abuse Counselor, the Educational Liaison, the Social Worker, and (if at all possible) the referring agency in order to design an individualized case plan. This plan will consist of short and long-term goals that will address each area found to be deficient, to include family therapy, individualized counseling, and educational-based classes. 

Educational-Based Classes

The following classes will be offered at The Bridge Center on a recurring basis: 

  • Anger Management 
  • Family Wellness 
  • Life Skills & Vocational Exploration 
  • Alcohol and Drug Prevention 
  • Chemical Dependence 
  • Positive Choices, Positive Futures 
  • Parent Support Group 

 A student will be placed in classes on an as-needed basis, determined by the evaluation process. 

Incentives/Sanctions 

The Bridge Center is strength-based with the ultimate goal of minimizing deficits. With that in mind, our staff will use natural consequences for ongoing negative behaviors (i.e. communication and involvement with parole/probations with instances of continued truancies). With instances of disruptive behavior in The Bridge Center, a student or student’s family will be asked to leave and reschedule a more appropriate time to return, extending the time of the treatment program. Part of the outcome-driven and supportive role that we play in a student’s re-engagement in their education is the incentives offered for achievements in the program. Our facilitators will work closely with the client and their family to procure incentives and rewards for those who are successfully working their case plan. These may include experiential rewards, points towards special activities, coupons, event tickets, and other incentives. Both the incentives and sanctions are utilized to encourage the client and his/her family to become re-engaged with one another, at school, and in the community.
